El Campo Memorial Hospital strives to make your hospital stay as comfortable and successful as possible, but the care they give does not stop there. El Campo Memorial Hospital’s Discharge Planner is responsible for helping the patient and family make the transition back home a positive one.

El Campo Memorial Hospital aids the patient and their family in developing a feasible post hospital plan of care with the understanding of the social, psychological, cultural, environmental, and financial factors which may affect the patient’s recovery. The Discharge Planner will follow-up with the patient/family members through personal contacts, questionnaires and telephone contacts. Referrals are available for home health care, support organizations, specialized health care facilities and outpatient programs.

El Campo Memorial Hospital Offers A New Program…

Swing Bed (Skilled Nursing / Rehabilitation)

If you have been hospitalized and are being discharged to a rehabilitation facility, you have the choice to come to El Campo Memorial Hospital for that REHAB Care!

The purpose of the Swing Bed Program is to allow a patient to stay in the hospital for a longer period of time and be under the observation and care of a physician and nurse.

The ECMH Discharge Planner and a Physician will evaluate the patient and if certain criteria are met, the patient/or significant other will be given the opportunity to decide to transfer over to this program. Current ECMH patients will not be changing rooms and the care provided will not be disrupted. Out of town hospitalized patients can be transferred to the ECMH Swing Bed Program by contacting the Discharge Planner.

ECMH’s Swing Bed Program is to assist patients in improving their physical, emotional and social function to become as independent as possible. Daily activities are provided to patients in the Swing Bed Program. ECMH Swing Bed Program is an alternative to short term Nursing Home Rehabilitation Care.

ECMH Swing Bed is a Medicare sponsored program that “swings” patients from acute care to skilled care who need temporary and transitional care.
