Community Home Care (CHC) is the only home health agency locally owned and operated in El Campo. This service of El Campo Memorial Hospital offers skilled nursing, physical therapy, occupational therapy, speech-language pathology, medical-social services, home health aide services to homebound individuals needing medical care, and educational services and therapy in their homes.  CHC is licensed by the Texas Department of Health, and is Medicare certified.  CHC provides services to patients 18 years or older, excluding obstetrics patients.

CHC professionals serve Wharton, Colorado and Jackson county areas, as well as parts of the Matagorda and Fort Bend counties.

Community Home Health services are covered by Medicare, Medicaid and most private insurances.  When your doctor prescribes home care, tell them you prefer CHC!

Community Home Care is licensed by the Department of Aging and Disability Services and is certified by Medicare and Medicaid.

  • Have a skilled care need on an intermittent basis
  • Have services ordered by a physician (i.e., doctor's referral)
